All play/grooming services are an additional charge. If you do not sign up for playtime, your pet will get a walk 3x per day only.

If illness or emergency occurs and no one can be reached, Ledgebrook Kennel and Woburn Animal Hospital staff are authorized to proceed
with any treatment deemed necessary to protect the health and well-being of your pet(s).

This form is considered a legal document. Please review all information carefully before signing.

All pets admitted must be current on vaccinations/test required by Ledgebrook Kennel and Woburn Animal Hospital policy. Records must be
provided prior to check-in or they will be done at the hospital’s current rates. The owner will be charged for any exam, vaccines, and any
additional diagnostic tests or treatments required under hospital policy to ensure the health and safety of all boarded pets.

If illness or emergency occurs while boarding and the emergency contact cannot be reached, medical treatment will be provided at Woburn
Animal Hospital’s prevailing rates to ensure the best care for your pet(s). Ledgebrook Kennel and Woburn Animal Hospital staff will use their
best judgment to determine the appropriate level of care.

Pets not picked up by 10:00 a.m. will be charged an additional day of boarding. Ledgebrook Kennel will charge the credit card on file for the
full balance due. In cases of abandonment or death, the same authorization applies to settle any remaining charges.

By signing below, I, the owner or caretaker of the pet(s), confirm that I have read, understand, and agree to all terms and conditions outlined
above. I authorize Ledgebrook Kennel and Woburn Animal Hospital to provide medical care as needed and agree to be financially responsible
for all services rendered.

I understand that Ledgebrook Kennel and Woburn Animal Hospital will take all reasonable precautions to ensure the safety, health, and comfort
of my pet(s). However, I acknowledge that boarding, group play, and animal handling involve inherent risks, including illness, injury, stress, or
unexpected death.

I agree that Ledgebrook Kennel, Woburn Animal Hospital, and their staff shall not be held liable for any injury, illness, loss, or death of my
pet(s) arising from causes beyond their control, including but not limited to pre-existing conditions, contagious disease exposure, natural causes,
or the actions of other animals.

I further agree to assume full financial responsibility for veterinary treatment deemed necessary during my pet’s stay and authorize such
treatment at the discretion of the attending veterinarian.
